الإجراء الموثق لإيصال النتيجة الحرجة مباشرة إلى الطبيب المسؤول أو الفريق المعالج خلال زمن محدد. يتضمن السجل النتيجة والوقت واسم المتصل واسم المستلم وتأكيد إعادة القراءة. وعند تعذر الوصول إلى أحد، يجب أن يكون مسار التصعيد مكتوباً مسبقاً.
Definition
The documented process of communicating a critical result directly to a responsible clinician or care team within a defined time. The record includes the result, the time, who called, who received it and the read-back. When no one can be reached, an escalation path must already be written down.
