التعريف
تقرير يصدر ليحل محل تقرير سبق إصداره وكان فيه خطأ، مثل نتيجة أو وحدة أو مريض خاطئ. يشترط ISO 15189 أن يُعلَّم بوضوح بأنه معدل، وأن يشير إلى التقرير الأصلي، وأن يبين التاريخ والشخص المسؤول. ويجب إبلاغ الطبيب لأن قراراً علاجياً ربما اتُّخذ بناءً على التقرير الأول.
Definition
A report issued to replace a released report that contained an error, such as a wrong result, unit or patient. ISO 15189 requires it to be clearly marked as revised, to reference the original and to show the date and the person responsible. The clinician must also be told, since a treatment decision may already have been made.
