إخفاق ضبط الجودة هو أي نتيجة كونترول أو نمط نتائج يخالف قواعد الرفض المعتمدة في المختبر، ومعناه أن الدورة التحليلية غير مقبولة. الاستجابة الصحيحة هي إيقاف إصدار النتائج والتحقيق في السبب، أما تكرار تمرير الكونترول حتى يدخل ضمن الحدود فيخفي أعطالاً حقيقية.
