قبل أن تبدأ: ثلاثة قراراتBefore you start: three decisions

من يقود الانتقال؟ شخص واحد من الفريق يملك القرار اليومي ويعرف الفحوص والأسعار، وغالباً هو رئيس الفنيين أو صاحب المختبر نفسه. أي حاسوب؟ حاسوب موثوق يبقى في المختبر ويعمل طوال ساعات الدوام، مع قرص خارجي للنسخة الاحتياطية. أي أسبوع؟ أسبوع عادي الحمل، بعيداً عن مواسم التحاليل المدرسية أو موسم الحج أو الأعياد.Who leads the transition? One person from the team who owns the daily decision and knows the tests and prices, usually the senior technologist or the owner. Which computer? A reliable PC that stays in the laboratory and runs through working hours, with an external disk for the backup. Which week? A week of ordinary load, away from school-test season, pilgrimage season or the holidays.

الأسبوع الأول: البيانات قبل النظامWeek one: data before the system

الخطأ الأكثر شيوعاً هو تنصيب النظام في اليوم الأول ثم اكتشاف أن قائمة الفحوص في رأس الفني وليست مكتوبة في أي مكان. اعكس الترتيب.The commonest mistake is installing the system on day one and then discovering that the test list lives in the technologist's head and is written nowhere. Reverse the order.

• قائمة الفحوص: اسم الفحص بالعربية والإنكليزية، الوحدة، المجال المرجعي بحسب العمر والجنس، نوع الأنبوب، الجهاز الذي يُجرى عليه، ووقت التسليم الموعود.The test list: the test name in Arabic and English, the unit, the reference range by age and sex, the tube type, the analyser it runs on, and the promised delivery time.
• قائمة الأسعار: سعر كل فحص والباقات والخصومات، وأسعار الجهات الدافعة إن اختلفت.The price list: each test's price, the panels and discounts, and payer prices if they differ.
• المرضى: الاسم بالعربية واللاتينية، تاريخ الولادة، الهاتف. الأعمدة الفارغة تُترك فارغة بدل ملئها بأرقام وهمية.Patients: the name in Arabic and Latin letters, date of birth, phone. Empty columns stay empty and are never filled with dummy values.
• الأطباء المحيلون والجهات الدافعة، بأسمائهم كما تريدها على التقرير والكشف.Referring doctors and payers, with their names as you want them on the report and the statement.

An hour of cleaning in Excel saves a week of correction inside the system. الأسبوع الثاني: التنصيب والحسابات والملصقاتWeek two: installation, accounts and labels

التنصيب نفسه ساعة: ملف واحد، ومعالج إعداد بالعربية يسأل عن هوية المختبر والمستخدم الأول والطابعات ومجلد النسخ الاحتياطي. بعده ثلاثة أعمال تحتاج إلى يومين:The installation itself takes an hour: one file, and an Arabic setup wizard asking for the laboratory's identity, the first user, the printers and the backup folder. After it, three jobs that need two days:

• حساب لكل فرد باسمه وصلاحياته. الاستقبال يسجّل ويفوتر، والفني يُدخل ويراجع، والمدير يعتمد. الحساب المشترك يمحو المسؤولية ويُرفَض من اليوم الأول.An account for each person in their own name with their permissions. Reception registers and invoices, the technologist enters and reviews, the manager releases. A shared account erases responsibility and is refused from day one.
• طابعة الملصقات: جرّب الملصق على أنبوب حقيقي في البراد ساعتين، وتأكد أن الباركود يُقرأ وأن الاسم لا يمحوه التكاثف.The label printer: test the label on a real tube in the fridge for two hours, and check the barcode reads and condensation does not wipe the name.
• قالب التقرير: الشعار والاسم والعنوان، وتوقيع المعتمد، ورمز التحقق. اطبع تقريراً واحداً وأعطه لطبيب تعرفه واسأله رأيه.The report template: logo, name and address, the releaser's signature and the verification code. Print one report, give it to a doctor you know and ask their opinion.

في نهاية الأسبوع، ربط الجهاز الأول. ابدأ بالجهاز الأكثر استخداماً، وأرسل طراز الجهاز إلى لاب سوريا قبل ذلك ليُجهَّز الربط. تظهر النتيجة الأولى من الجهاز على الشاشة من غير كتابة، وتكون هذه لحظة اقتناع الفريق.At the end of the week, connect the first analyser. Start with the busiest one, and send the model to LabSyria beforehand so the link is prepared. The first result appears on the screen from the analyser with no typing, and that is the moment the team is convinced.

الأسبوع الثالث: تشغيل مزدوجWeek three: parallel running

كل مريض يُسجَّل على الورق كما كان، وفي النظام أيضاً. التقرير الذي يخرج للمريض ما زال تقرير الطريقة القديمة. في المساء يقارن قائد الانتقال بين الاثنين: عدد المرضى، والفحوص، والمبالغ.Every patient is recorded on paper as before, and in the system too. The report the patient receives is still the old-method report. In the evening the transition lead compares the two: patient counts, tests and amounts.

الفروق التي تظهر تكون من ثلاثة أنواع: فحص سُمّي بطريقتين، سعر قديم في الدفتر وجديد في النظام، أو مريض سُجّل مرتين. كل فرق يُصحَّح في النظام في اليوم نفسه. يوم الجمعة من هذا الأسبوع تكون المقارنة بلا فروق، أو تمدّد الأسبوع.The differences that appear are of three kinds: a test named two ways, an old price in the ledger and a new one in the system, or a patient registered twice. Each is corrected in the system the same day. By the Friday of this week the comparison shows no differences, or the week is extended.

الأسبوع الرابع: الاعتماد وقواعد الورق الجديدةWeek four: go-live and the new rules for paper

من صباح الأول من الأسبوع الرابع يخرج التقرير من النظام، ويُفرج عنه الفني باسمه، ويحمل رمز التحقق. ضبط الجودة يُسجَّل في النظام، وإذا فشل توقّف الإفراج حتى يُسجَّل الإجراء. يعلّق قائد الانتقال ورقة واحدة قرب الاستقبال: ثلاث قواعد للورق.From the morning of the first day of week four the report leaves the system, is released by the technologist in their name and carries the verification code. Quality control is recorded in the system, and if it fails release stops until the action is recorded. The transition lead pins one sheet by reception: three rules for paper.

• دفتر الطوارئ: حين يتعطّل الحاسوب يُسجَّل المريض على ورقة مرقّمة، ويُدخَل في النظام في اليوم نفسه بعد عودته، بالتاريخ الحقيقي.The emergency ledger: when the PC fails the patient is recorded on a numbered sheet, and entered into the system the same day after it returns, with the real date.
• لا تقرير مكتوب باليد يخرج للمريض بعد الاعتماد مهما كان الازدحام.No handwritten report leaves for the patient after go-live, however busy the day.
• النسخة الاحتياطية تُفحَص كل يوم اثنين: يفتح المدير مجلد النسخ ويرى أن ملف البارحة موجود، ومرة في الشهر يُستعاد على حاسوب آخر.The backup is checked every Monday: the manager opens the backup folder and sees last night's file is there, and once a month it is restored on another PC.

بعد الشهر: ما الذي تكسبهAfter the month: what you gain

في نهاية الشهر الأول يملك المختبر ما لم يملكه قط: عدد مرضاه الحقيقي في اليوم، ومتوسط زمن التسليم لكل فحص، والفحوص التي تدرّ أكثر، والجهات الدافعة التي تتأخر في الدفع. وكل تقرير خرج يمكن التحقق منه برمز، وكل نتيجة تعرف من اعتمدها. هذه هي الحوكمة من دون أن تُسمّى.At the end of the first month the laboratory owns what it never had: its true daily patient count, the average turnaround per test, the tests that earn most, and the payers that pay late. Every report that left can be verified by its code, and every result knows who released it. That is governance without the name.
