قبل أن نبدأ: ما هو النظامBefore we start: what the system is

النظام برنامج يُنصَّب على حاسوب المختبر بنظام ويندوز من ملف واحد، ويعمل كخدمة في الخلفية، وتفتحه شاشات المختبر الأخرى وهواتف الفريق من المتصفح على شبكة المختبر. لا اشتراك سحابي ولا خادم في الخارج ولا إنترنت في العمل اليومي. الواجهة عربية من اليمين إلى اليسار، والإنكليزية بضغطة. ويوزّعه ويدعمه فريق لاب سوريا في سوريا. ليس هو برنامج محاسبة عام، ولا نظام مستشفى كامل، بل نظام مختبر: مرضى وطلبات وعيّنات ونتائج وتقارير وإحالات وحسابات مختبر.The system is a program installed on the laboratory's Windows PC from one file, running as a background service, opened by the laboratory's other screens and the team's phones from a browser on the laboratory's network. No cloud subscription, no server abroad, no internet in the daily work. The interface is Arabic, right to left, with English one click away. It is distributed and supported by the LabSyria team in Syria. It is neither a general accounting package nor a full hospital system; it is a laboratory system: patients, orders, specimens, results, reports, referrals and a laboratory's accounts.

الثامنة صباحاً: الاستقبالEight in the morning: reception

تفتح موظفة الاستقبال شاشة «طلب جديد». تكتب رقم هاتف المريض فيظهر إن كان مسجّلاً، أو تسجّله بالاسم وتاريخ الميلاد ورقم الهاتف؛ لا بريد إلكتروني، فالمريض السوري يُسجَّل برقم جوّاله. تختار الفحوص من دليل المختبر أو لوحة جاهزة، وظائف الكبد مثلاً، فيظهر السعر بالليرة بسعر صرف اليوم الذي حدّده المدير صباحاً، والأنابيب المطلوبة لكل فحص، وأي فحص يُجرى في المختبر وأي فحص يُحال إلى مختبر مرجعي. تعطي خصماً إن كان ضمن الحد المسموح لها، وتختار طريقة الدفع: نقداً، أو سيرياتيل كاش، أو إم تي إن كاش، أو على حساب شركة أو جهة تأمين.The receptionist opens New order. She types the patient's phone number and they appear if registered, or she registers them with name, date of birth and phone; no email, because the Syrian patient is registered by mobile number. She picks tests from the laboratory's catalogue or a ready panel, liver function say, and the price appears in pounds at today's exchange rate the manager set in the morning, with the tubes each test needs and which test is done in house and which is referred to a reference laboratory. She gives a discount if it is within the limit she is allowed, and chooses the payment: cash, Syriatel Cash, MTN Cash, or a company or insurer account.

If the order includes a referred test, the referral is created and its tubes registered for dispatch, with nothing re-entered.

التاسعة: العيّناتNine: the specimens

يسحب الفني العيّنة ويلصق الملصق. من هذه اللحظة يحمل الأنبوب معرّفاً فريداً بصيغة تحمل رمز المختبر واليوم وخمسة أحرف لا تتكرر، فلا يلتقي أنبوبان من مختبرين بالرقم نفسه. حقل المسح في أعلى كل شاشة يقرأ الملصق أو رقم القيد أو كشف الإرسال ويفتح الشاشة المناسبة؛ ماسح USB يعمل كلوحة مفاتيح، أو كاميرا الهاتف. كل انتقال للأنبوب يُكتب في سلسلة عهدة لا تُعدَّل. الأنبوب المنحل أو المتخثر أو الناقص يُرفض بسبب من القائمة، فتظهر الزيارة في قائمة العمل تحت «عيّنات تنتظر إعادة السحب».The technician draws the specimen and sticks the label on. From that moment the tube carries a unique identifier in a form that holds the laboratory's code, the day and five characters that never repeat, so two tubes from two laboratories never meet under the same number. The scan field at the top of every screen reads the label, the accession number or a manifest and opens the right screen; a USB scanner works as a keyboard, or the phone's camera. Every move of the tube is written to a custody chain that cannot be edited. A haemolysed, clotted or short tube is rejected with a reason from the list, and the visit appears on the worklist under specimens awaiting recollection.

العاشرة: النتائجTen: the results

أجهزة الكيمياء والدم والمناعة المربوطة عبر بوابة الأجهزة ترسل نتائجها تلقائياً، فتصل إلى ملف المريض مطابقةً برمز الأنبوب، ومعها الجهاز والمشغّل ودفعة الكاشف. نتيجة وصلت من جهاز ولم تجد طلباً تظهر في قائمة العمل لتُربط بزيارتها. الفحوص التي تُجرى يدوياً تُدخل في شاشة الزيارة، حيث تكون الفحوص المطلوبة ووحداتها ومجالاتها المرجعية جاهزة، أو في ورقة العمل: فحص واحد عمودياً لكل الزيارات التي تنتظره، مع القيمة السابقة لكل مريض بجانبه. النتيجة قد تكون رقماً أو كلمة: سلبي، إيجابي، +، ++. والزرع والتحسس يُدخلان كعضويات ومضادات حيوية ويُطبعان جدولاً.Chemistry, haematology and immunoassay analysers connected through the analyser gateway send their results by themselves; they land in the patient's file matched by the tube's barcode, with the analyser, the operator and the reagent lot attached. A result that arrived from an analyser and found no order appears on the worklist to be attached to its visit. Tests done by hand are entered on the visit screen, where the ordered tests, their units and their reference ranges are ready, or on the worksheet: one test down one column for every visit waiting for it, with each patient's previous value beside it. A result may be a number or a word: negative, positive, +, ++. Cultures are entered as organisms and antibiotics and print as a table.

الحادية عشرة: التحقق والتوقيعEleven: the checks and the signature

قبل أن يوقّع المعتمِد، يعمل النظام لحسابه. الفحوص المحسوبة مثل LDL وeGFR تُحسب من مدخلاتها. الفحوص الانعكاسية تُضاف حين تستدعيها نتيجة، FT4 بعد TSH خارج المجال مثلاً. فحص الفروق يقارن القيمة الجديدة بآخر قيمة معتمدة للمريض ويوقف ما قفز أكثر مما تسمح به القاعدة. القيم الحرجة تنتظر إقراراً باسم من اطّلع عليها. وبوابة ضبط الجودة ترفض التوقيع إن كان الجهاز موقوفاً أو فشل آخر ضبط له، وتنبّه إن لم يُجرَ ضبط ضمن النافذة أو إن تغيّرت دفعة الكاشف بعد آخر ضبط ناجح. قائمة الانتظار مرتّبة بالحرج أولاً، ثم الخارج عن المجال. المعتمِد يرى كل نتيجة أمام مجالها وقيمتها السابقة، ويضيف تعليقاً من مكتبة العبارات أو بحرية، ويوقّع باسمه.Before the signer signs, the system works on their behalf. Calculated tests such as LDL and eGFR are worked out from their inputs. Reflex tests are added when a result calls for them, FT4 after an out-of-range TSH say. The delta check compares the new value with the patient's last signed one and holds what jumped more than the rule allows. Critical values wait for an acknowledgement under the name of who looked. The quality gate refuses the signature if the analyser is locked out or its last control failed, and warns if no control ran in the window or the reagent lot changed after the last passing control. The queue is ordered by critical first, then out of range. The signer sees every result against its range and previous value, adds a comment from the phrase library or freely, and signs under their own name.

الظهر: التقريرNoon: the report

التقرير مصمَّم ليُقرأ. الترويسة تحمل شعار المختبر واسمه بلغتين وعنوانه وأرقامه، وعنوان التقرير ورقم القيد وشريطاً رمزياً له يقرؤه ماسح الاستقبال. سطر ملخّص يقول كم فحصاً خرج عن المجال ويسمّيها، بشريط أحمر إن كانت قيمة حرجة. كل فئة جدول: اسم الفحص بلغتين، والقيمة، والوحدة، والمجال المرجعي، وشريط صغير يبيّن موقع القيمة فيه، والتقييم بالكلمات، والقيمة السابقة بتاريخها. ملاحظة بلغة المريض تحت الفحص حين يحملها الدليل. الفحوص المحالة تذكر أنها أُجريت في المختبر المرجعي. وبجانب التوقيع رمز QR يفتح نتائج هذه الزيارة على الهاتف، وفي التذييل رمز آخر للتحقق من أن الورقة أصلية.The report is designed to be read. The header carries the laboratory's logo, its name in both languages, its address and numbers, the report title, the accession number and a barcode of it the front-desk scanner reads. A summary line says how many tests are out of range and names them, on a red band if a value is critical. Every category is a table: the test name in both languages, the value, the unit, the reference range, a small bar showing where the value sits, the flag in words, and the previous value with its date. A note in the patient's language under a test when the catalogue carries one. Referred tests say they were performed at the reference laboratory. Beside the signature sits a QR code that opens this visit's results on a phone, and in the footer another to check that the paper is genuine.

من شاشة النتائج يطبع الفريق التقرير مباشرة على الطابعة المعيّنة، أو يرسله عبر نافذة المشاركة في ويندوز إلى واتساب كملف، أو يسجّل أنه سُلّم باليد أو أُبلغ هاتفياً. كل تسليم يُكتب في الزيارة. والطبيب المُحيل له نسخته من الزر نفسه.From the results screen the team prints the report straight to the assigned printer, sends it through the Windows share sheet to WhatsApp as a file, or records that it was handed over or read out by phone. Every delivery is written on the visit. The referring doctor gets their copy from the same button.

المساء: المريض وهاتفهEvening: the patient and the phone

يفتح المريض صفحة النتائج من الرابط على الإيصال، يدخل الرمز وتاريخ ميلاده، أو يمسح رمز التقرير الورقي فيكفيه تاريخ الميلاد. ما دام العمل جارياً ترى الصفحة له المراحل الأربع والموعد المتوقع. بعد الاعتماد يقرأ النتائج كما في التقرير، ويفتح ملف PDF ويحفظه ويشاركه مع طبيبه، ويجد زر واتساب للمختبر. الرابط لا يعرض شيئاً لمن لا يملك تاريخ الميلاد، ويُوقف بعد محاولات خاطئة. ويمكن لموقع المختبر أن يحمل صندوق النتائج نفسه بسطر واحد.The patient opens the results page from the link on the receipt, enters the code and their date of birth, or scans the paper report's code, where the date of birth alone is enough. While the work is in progress the page shows them the four steps and the expected time. After signing they read the results as on the report, open the PDF, save it, share it with their doctor, and find a WhatsApp button to the laboratory. The link shows nothing to anyone without the date of birth, and locks after wrong attempts. The laboratory's own website can carry the same results box with one line.

في الأثناء: الإحالة إلى المختبر المرجعيMeanwhile: the referral to the reference laboratory

الفحص الذي لا يُجرى في المختبر صار إحالة منذ تأكيد الطلب. الإحالة ملف موقّع بمفتاح المختبر يرافق الأنابيب على ذاكرة USB أو يُرسل عبر واتساب، أو يذهب عبر مركز لاب سوريا إن كان المختبر مربوطاً به: صندوق بريد مغلق يحمل الملفات ولا يستطيع فتحها، مع مزامنة تنتظر عودة الاتصال. الأنابيب تُجمع في كشف إرسال مختوم برقم ودرجة حرارة، والناقل يمسحه من هاتفه عند الاستلام والتسليم. في المختبر المرجعي يصير الملف زيارة خاصة به وتُسجَّل الأنابيب واردة، وحين يوقّع، تعود النتائج ملفاً موقّعاً إلى زيارة المريض نفسها عندك، وتمرّ بالمراجعة والتوقيع المعتادين. الأنبوب الذي رُفض هناك يعود في الملف نفسه بسببه.A test not done in house became a referral the moment the order was confirmed. The referral is a file signed with the laboratory's key that travels with the tubes on a USB stick or over WhatsApp, or goes through the LabSyria hub if the laboratory is connected to it: a blind mailbox that carries files and cannot open them, with a sync that waits for the connection to return. The tubes are gathered into a manifest sealed with a number and a temperature, and the courier scans it from a phone at pickup and delivery. At the reference laboratory the file becomes a visit of its own and the tubes are registered as incoming, and when it signs, the results return as a signed file into the same patient visit at your laboratory and pass through the usual review and signature. A tube rejected there returns in the same file with its reason.

ولأن المختبرات تتحاسب، يحمل ملف النتائج سعر كل فحص من قائمة أسعار المختبر المرجعي، فيُقيَّد على الطرفين في اليوم نفسه. آخر الشهر يصدر المختبر المرجعي كشفاً موقّعاً لكل مختبر مرسِل، يستورده الآخر ويرى المطابقة سطراً سطراً، ويقرّ أو يعترض على سطر، ويُسجَّل التسديد عند الطرفين.And because laboratories settle with each other, the results file carries each test's price from the reference laboratory's price list, so it is booked on both sides the same day. At the end of the month the reference laboratory issues a signed statement per sending laboratory, which the other imports and checks line by line, acknowledging or disputing a line, and the settlement is recorded on both sides.

آخر اليوم: الصندوق والحساباتEnd of the day: the cash book and the accounts

يغلق الصرّاف ورديته فيصدر تقرير الصندوق بحسب طريقة الدفع والصرّاف. الفواتير المفتوحة تظهر في قائمة المدينين. حسابات الجهات، الشركات وشركات التأمين والعيادات التي ترسل مرضاها على الحساب، لها شاشتها: الذمم وأعمارها، وقبض يوزَّع على أقدم الفواتير، وشطب بسبب، وتذكير واتساب، وكشف شهري يُطبع. والأطباء المُحيلون لهم كشفهم الشهري بزياراتهم ونصيبهم.The cashier closes the shift and the cash report comes out by payment method and cashier. Open invoices appear in the debtors list. Payer accounts, the companies, insurers and clinics that send their patients on account, have their own screen: balances and their ageing, a receipt spread over the oldest invoices, write-offs with a reason, a WhatsApp reminder, and a printable monthly statement. The referring doctors have their monthly statement with their visits and share.

ما يراه المديرWhat the manager sees

الرئيسية مشغّل لكل الشاشات مقسّمة بحسب القسم، مع أعداد ما ينتظر. لوحة اليوم تعرض مؤشرات اليوم وما يحتاج قراراً: نتائج تنتظر المراجعة، تقارير تنتظر الاعتماد، عيّنات تأخرت عن موعدها، أجهزة لم تتصل. شاشة التقارير تعطي أرقام المدة: الزيارات والمفوتر والمقبوض وغير المسدَّد، والفحوص حسب الإيراد، والأطباء حسب الزيارات، وزمن الإنجاز لكل فحص، الوسيط والتسعون بالمئة، مقارنةً بما وُعد به المريض في الدليل، ونسبة نجاح ضبط الجودة مع عدد التشغيلات خلفها. وشاشة النمو تقرأ إحالات المختبر نفسه وتقول أي فحص يستحق جهازاً في المختبر، وكم يبقى من العمل في الداخل شهرياً لو اشتُري.Home is a launcher of every screen by section, with the counts of what is waiting. The Today board shows the day's indicators and what needs a decision: results awaiting review, reports awaiting signature, specimens past their promised time, analysers that have not connected. The reports screen gives the period's figures: visits, billed, collected and outstanding, tests by revenue, doctors by visits, turnaround per test, median and 90th percentile, against what the patient was promised in the catalogue, and the quality-control pass rate with the run count behind it. The growth screen reads the laboratory's own referrals and says which test deserves an analyser in house, and how much work would stay in house each month if it were bought.

حين ينقطع كل شيءWhen everything else stops

لا شيء في العمل اليومي يحتاج إلى الإنترنت. حين تنقطع الكهرباء، حزمة نماذج ورقية مرقّمة من تسلسل النظام تُطبع مسبقاً، وتُدخل أرقامها حين تعود. قاعدة البيانات مكتوبة بحيث لا يضيع ما تأكّد حفظه عند انقطاع مفاجئ، والخدمة تعود وحدها مع الحاسوب. نسخة احتياطية من قاعدة البيانات والمرفقات والإعدادات تُؤخذ كل ليلة مشفّرة بكلمة سر المختبر، وتُحفظ أربعة عشر يوماً، ويتحقق منها زر واحد. التحديثات تصل ملفات موقّعة، عبر الإنترنت أو على ذاكرة USB، ولا يقبل النظام الرجوع إلى إصدار أقدم. وحزمة الدعم زر واحد يجمع ما يسأل عنه فريق لاب سوريا أولاً، مع حجب كل سر ومن غير أي سجل مريض.Nothing in the daily work needs the internet. When the power fails, a pack of paper forms numbered from the system's own sequence is printed in advance, and the numbers are entered when it returns. The database is written so that nothing confirmed as saved is lost in a sudden cut, and the service comes back with the PC by itself. A backup of the database, attachments and settings is taken every night, encrypted with the laboratory's passphrase, kept for fourteen days, and verified with one button. Updates arrive as signed files, online or on a USB stick, and the system never goes back to an older version. The support bundle is one button that gathers what the LabSyria team asks for first, with every secret blanked and no patient record.

البدايةGetting started

التنصيب ملف واحد على حاسوب ويندوز عادي، ثم معالج إعداد بالعربية: اللغة، دور المختبر، هويته وشعاره، المستخدم الأول، عنوان الشبكة، الطابعات، ملف الترخيص، الربط بالمركز، ومجلد النسخ الاحتياطي. من النظام القديم تُستورد المرضى والأطباء والنتائج السابقة والأرصدة من ملفات Excel أو CSV بعناوين عربية أو إنكليزية وتواريخ سورية، بمعاينة تسمّي كل صف فيه مشكلة قبل أي استيراد. ولمن يريد أن يرى كل شيء قبل أن يقرر، مختبر عرض يُحمَّل في دقيقة: شهر كامل من عمل مختبر افتراضي بمرضاه وزياراته وتقاريره وإحالاته وحساباته. ثم التدريب مع فريق لاب سوريا، ودليل المستخدم العربي داخل النظام يعمل بلا إنترنت ويُطبع، والأكاديمية المجانية لبقية الفريق.Installation is one file on an ordinary Windows PC, then an Arabic setup wizard: language, the laboratory's role, its identity and logo, the first user, the network address, the printers, the licence file, the hub connection, and the backup folder. From the old system, patients, doctors, past results and balances are imported from Excel or CSV files with Arabic or English headers and Syrian dates, with a preview that names every problem row before anything is imported. For those who want to see everything before deciding, a demonstration laboratory loads in a minute: a full month of a fictional laboratory's patients, visits, reports, referrals and accounts.
